Send a Sample
Submit a test order form electronically (you will receive an emailed copy) and follow the instructions on this page to send your sample(s).
Collecting the Sample
- Draw blood into a serum vacutainer (either a red top tube or an SST) invert tube 8 to 10 times and allow blood to clot for 60 minutes.
- Spin clotted blood at 1500 x G for 10mins.
- Transfer a minimum of 0.5 ml serum into an appropriate transport container and freeze as soon as possible at a temperature between -65 and -85 degrees C. Larger volumes are preferable to allow for possible duplicate analysis (if necessary).
- Complete this Test Requisition Form for each sample and enclose with the shipment. If samples are received from a submitting client using another form that contains all of the required information, ATD Laboratory will transfer the data from the client’s form to the ATD Requisition Form and will maintain both documents. An ATD Test Requisition will be completed for each sample received.
- Ensure any special instructions are noted in the Special Instructions/Notes section of the form.
Packing Information
Sample vials should be well padded and placed in a secondary container (i.e. ziplock bag). ALL samples must be labeled with the two identifiers outlined in the Test Requisition Form.
Shipping Information
Please do not ship samples on Fridays or before a holiday. Please ship with a carrier that is HIPAA and DOT certified – overnight on dry ice to:
